Payment Options

 

  • Monthly payments are based on patient age as of date of enrollment into practice and are updated on January 1st each year.

    • Children (ages 0-18 years)

      • $50/mos. with adult family member

      • $75/mos. without adult family member

    • Young Adults (ages 19-26 years) $50/mos.

    • Adults ages 27-39 years $75/mos.

    • Adults ages 40-59 years $100/mos.

    • Adults ages 60-64 $115/mos.

    • Anyone who qualifies for Medicare is not eligible to participate in Direct Primary Care Membership. For patients with traditional Medicare insurance, Simplicity bills Medicare for visits.

  • Maximum Monthly Household/Family Membership Fee is $300/mos (up to 6 people). Additional people after 6 are $40/month.)

  • Simplicity accepts and bills

    1) Premera and Lifewise commercial insurance

    2) United Health Care commercial and medicare advantage part C plans

    3) Traditional Medicare Part B plans plus any supplemental Part G or Medigap plans.
